To become a distributor, you will need a "Reseller Number". This is the same as a State issued "Tax Registration Number" or Universal Business Identification (UBI) number. You obtain this number through your state Department Of Licensing. In WA State, you may apply for this number by completing the Master Business License application at http://www.dol.wa.gov/business/file.html. (If you are already a business, you should already have this number.)

Once you have your tax registration number, fill out the resale certificate from the Dept. of Revenue (for WA state, go online at, http://dor.wa.gov/Docs/Forms/ExcsTx/ExmptFrm/ResaleCertificate_E.pdf. (Once completed, print the form and an extra copy for your own records.)
Enter your Tax Registration Number for question 4 on the resale certificate.
The first box on the resale certificate's lower section ("For resale in the regular course of business without intervening use" is the only box you will need to check.
Once you have the resale certificate filled out, go to my website at http://dietadvisor.stemtechbiz.com/Default_US_EN.aspx and click "Join". This will bring up the distributor application which asks for the reseller number at the bottom of page one. You will need to send a copy of the resale certificate to StemTech by mail, fax or email for their records. Obtain their forwarding information from StemTech customer service at 949-542-8600 if it isn't on the form.

When filling out the distributor application, you are offered several set-up options.

If you want a convenient way to sign up other distributors under you, I would recommend the "FastStart Pack". The FastStart Pack requires an initial order of 6 bottles of StemEhance, and includes brochures, DVD promo materials, order forms and other info, and a free StemTech distributor website, which is really handy for retail orders or for others to become distributors under you because it's self-serve and you don't have to fill out any paperwork. The total cost of the FastStart Pack was $336.48 for me here in Washington State.

If you just want to be a distributor mainly for your own use, you can go with the first option in the list, "Distributor Kit $25.00", which will require a $25.00 setup fee and no initial product order.

All distributor accounts get commissions and rebates on non-autoship orders, and all accounts are assessed a $25.00 annual renewal fee.

Autoship orders are automatically shipped to you once a month. They can be as little as one bottle a month, and discontinued or restarted at will. The cost of autoship orders is $5.00 less than the wholesale price. (If you have a website and stop your autoship orders, you will be assessed a $9.95/month fee for website maintenance.)
